ENROLL IN THE PATIENT PORTAL

Hospital Portal

There are two ways to enroll in the Hospital Portal:

1. When you are registering for an appointment, add your email address to your medical record.  Ask for a portal instruction pamphlet with your medical record number inside.

2. Call (802) 447-5687 to add your email address to your medical record and be provided your medical record number.

Once your email address is filed, you will be auto-enrolled and will receive an email from the Patient Portal with further instructions for setting up your account.

Medical Practice Portal

To enroll in the Medical Practice Portal, you will need to go to your physician's office and obtain a letter with PIN number. For more information, please call (802) 440-6000, option 2.
