PATIENT PORTAL

SVMC's interactive web portals give patients and their families easy, secure access to their health information. The portals also provide quick, online communication with staff. Because the information comes directly from your medical record, you can be sure it’s up-to-date.

SVMC uses two seperate portals for patients accessing hospital services and those using medical pratices/offices. Use the links below to sign in to the Hospital Patient Portal or SVMC Practices and Affiliates Patient Portal, which includes our medical offices and orthopedic practices.

For help accessing your health information through either the Hospital Portal or the Medical Practices Portal, call (802) 440-6000 or email contact_us_mp@svhealthcare.org.

If this is a medical emergency, dial 911 or go to the nearest emergency room.

The Hospital Patient Portal has information in your hospital medical record, including information from:

  • Your inpatient stay
  • Your emergency department visit
  • Laboratory tests
  • X-ray or other imaging tests
  • Sleep testing
  • Other tests conducted at SVMC

The Medical Practice Portal has information from SVMC's Medical Practices, including:

In addition to handling your own care, SVHC allows parents and healthcare proxies with authorized consent to access the records of those individuals for whom they manage care. You can request proxy access for the portal by completing the appropriate form below and returning to the Health Information Services department.
