Transferring Patients to SVMC
Southwestern Vermont Medical Center (SVMC) is a community hospital that will accept patients in transfer[1] from various settings who fall within the scope of services provided.[2]
The first step is to call the Dartmouth Health (DH) Capacity Coordination Center at
877-999-9870. You will speak with a staff member who will consider options for your patient.
You may then be connected with an SVMC Administrative House Supervisor who will
- Gather clinical and basic demographic information about the patient,
- Locate an accepting physician[3] and arrange for an inpatient bed, and
- Place a return call to the DH Capacity Coordination Center within 20 minutes to confirm acceptance, the name of the accepting physician, and the room assignment.
Before transfer, please utilize the DH Capacity Coordination Center number (877-999-9870) to ensure call capture via the recorded lines. For communication after the patient arrives at SVMC, the direct line to the SVMC transfer coordinator is 877-903-SVMC (7862). The fax number for sending medical records to SVMC is 802-440-8202.
