Todd Salvesvold Welcomed as Bennington Blueprint’s Community Health Team Leader

BENNINGTON, VT—February 14, 2019—The Vermont Blueprint for Health is pleased to add Todd Salvesvold, RN, to its Bennington team. He joins as the group’s community health team leader. In this role, he is an employee of Southwestern Vermont Health Care.

“Todd is the perfect candidate for this position,” said Kristi Cross, DNP, RN, director of the Bennington Blueprint. “He has a dynamic personality and the cooperative spirit that this position requires. We are so excited to welcome him as a part of this important team.”

Salvesvold holds a bachelor’s in psychology from Sage College and a master’s in community psychology and chemical dependency from Sage Graduate School, both in New York. In 2011, he received his associates in nursing from Samaritan School of Nursing and became a registered nurse.

Since 2013, Salvesvold has worked as a supervisor in the medical affairs case management department of Capital District Physicians Health Plan (CDPHP) in Albany, NY. He has worked as a case manager and nurse at Glens Falls Hospital Center for Recovery and St. Peter’s Health Partners at Samaritan Hospital, both in New York. From 2007 – 2011, he was a case manager and emergency psychiatric clinician with the United Counseling Service in Bennington, VT.

“I am very excited to be part of Bennington Blueprint and Southwestern Vermont Health Care,” Salvesvold said. “I look forward to working with together with colleagues and partners in support of its initiatives.”

The Vermont Blueprint for Health is a nationally recognized initiative that designs community-led strategies for improving health and well-being. The Blueprint invests in and supports patient-centered medical homes, community health teams, community collaboratives, the hub and spoke system of opioid addiction treatment, Support and Services at Home (SASH), the Women's Health Initiative, self-management and healthier living workshops, and a series of learning labs for communities and teams. Blueprint interventions have been shown to reduce the growth in health care costs while maintaining or improving health outcomes.
