Lost vaccination card?

Vaccination cards are becoming more important, as several venues are requiring proof of vaccination. Here’s what to do if you have lost your card:

• Call the SVMC COVID-19 Info Hotline at 802-440-8844.
• The nurse on the hotline will need your name, date of birth, and your phone number.
• Our team will reproduce your card for you and provide instructions regarding where you can pick up your new card as early as the next day.
