Early Interventionist, Part Time, Flexible Hours
Southwestern Vt Medical Cntr, FIT Program
Part Time (50 hrs per pay period)

Responsibilities:
The Early Interventionist will serve as a member of the core team(s) for the Region within the Community Resource Parent(s), School District Representative(s) and CSHN Social Worker. Will provide interim or ongoing service coordination for some of the referred infants and toddlers and their families and provide appropriate resource and referral information for families regardless of eligibility for service. Will facilitate and/or perform screening, initial evaluations or ongoing assessment of development as needed; perform outreach and education activities for families, advocacy groups, community agencies, programs, and organizations in coordination with other Core Team/Regional Team members to facilitate public awareness of early intervention services/supports and the referral process, and to strengthen collaborative relationships with community resources and supports.

Requirements:
BS in Special Education, Early Education, Physical or Occupational Therapy, Speech, Social Work, Nursing, Psychology, or other related field. Current Vermont certification, registration, or licensure as it applies. At least 2 years of experience providing direct services to children under the age of 3 with disabilities or special needs and knowledge of developmental process for young children; Knowledge of community resources and the process for accessing them; experience developing and writing family or individual service plans.
