When it comes to primary care, close to home, we’ve got our community covered. From pediatrics to family and internal medicine, the providers at SVMC Northshire Campus are here for you, at all stages of life.  

Recognized by the National Committee for Quality Assurance (NCQA) as a Patient Centered Medical Home, we provide a new model of personal, efficient, and effective primary care. Patients have access to several services not often available in doctor’s offices, including behavioral health services, diabetes education, and nutrition counseling, among others. Services include:

  • Routine, acute and chronic medical care 
  • Pediatric care, including consultations on child development, newborn care, well child and adolescent care, acute illness visits, immunizations, asthma management, ADHD, weight management and nutrition, contraceptive management, and behavioral health 
  • On-site blood draw services (by appointment)
  • On-site X-ray services (by appointment)
  • On-site physical, occupational and speech therapy
  • Medicare Annual Wellness Visits with a registered nurse 
  • Telehealth appointments: Directions and details on accessing telehealth visits will be provided prior to a scheduled appointment; insurance and state restrictions may apply
  • Our Patient Portal is a free service that offers secure messaging, appointment requests, online billing, access to medical forms and review of personal health information 
  • Preferred language care through electronic language services
  • Financial resources for patients can be found at this link or by calling 802-447-4500
  • SVMC's Patient Advocate can be reached at 802-442-4054 to discuss any concerns you have about your care that can't be resolved by the clinical practice team

A provider is on call for urgent (non-emergent) issues 24 hours a day. When the office is closed, an answering service will quickly relay your message to the provider on call. After you have made contact with the answering service, leave your phone line open so the on-call provider may return your call without delay. In the event of an emergency, please call 911.
