SVHC's interactive web portals give patients and their families easy, secure access to their health information. They also provide quick, online communication with staff. Because the information comes directly from your medical record, you can be sure it’s up-to-date.

We support our patients having access to their health information. Beyond the Patient Portal, we have also enabled the ability for mobile apps to provide you access to your health information in a safe and secure way. However, we are unaware of any apps that have been configured to connect to our system at this time. Our website will be updated when we become aware of any apps that you can use to access your health information.

Accessing a Relative's Information

In addition to handling your own care, SVHC allows parents and healthcare proxies with authorized consent to access the records of those individuals for whom they manage care. To get access for a relative you must set up a health care proxy. Call (802) 447-5687 to learn how to do this.

For Help by Phone
