LPN Home Care and Hospice - Part Time
VNA & Hospice of SVMC
Part-time (48 hrs per pay period)

Responsibilities:
The LPN provides skilled nursing assessment, planning, and care to home care and hospice patients and their families in order to maximize the comfort, well-being and dignity of patients and families consistent with VNA & Hospice policies and procedures. Provides supportive care and appropriate services in accordance with the attending physician's orders and interdisciplinary team plan of care.

Requirements:
Completion of LPN course from an accredited school. One year of LPN work experience required. Successful completion of NAPNES course on pharmacology and administration of medicines or completion of a similar course of study included in his/her nursing school curriculum. Current LPN Vermont license required and current CPR certification. Current driver's license and reliable transportation required.
