RN Home Care and Hospice; Weekends & Holidays
VNA & Hospice of SVMC
PT (48 hrs per pay period)
Responsibilities:
Provision of skilled nursing assessment, planning, and care to home care and hospice patients and their families in order to maximize the comfort, well-being and dignity of patients and families consistent with VNA & Hospice policies and procedures. Provides supportive care and appropriate services in accordance with the attending physician's orders and interdisciplinary team plan of care.
Requirements:
A.S. in Nursing, BSN preferred. Two years varied work experience as a professional nurse; preferably one of the two years in an acute care setting. Current license to practice as a registered professional nurse in the State of Vermont. Valid driver's license; reliable transportation. Current license/eligible to practice as a Registered Nurse in the State of Vermont. Home Health and/ Med-Surg experience preferred. Valid driver's license and reliable transportation.
